Description
Running longitudinally along the posterior cervical spine, the longissimus cervicis occupies the intermediate column of the erector spinae, positioned lateral to spinalis cervicis and medial to iliocostalis cervicis. Its fascicles ascend superomedially from the upper thoracic transverse processes toward the posterior tubercles of the cervical transverse processes, typically spanning T1 to T5 up to C2 to C6. Medial to these bundles, the cervical spinous processes form a midline ridge, while the occiput and posterior cranial sutures sit superiorly as stable osseous context. Inferiorly, the superior angles of the scapulae may enter the field, marking the transition toward the upper back. Posterior cervical extensor anatomy matters when you need to distinguish muscular pain from facet-mediated pain and radicular complaints, and the longissimus cervicis is a frequent contributor to postural overload and whiplash-associated myofascial symptoms. Palpation and needling targets in the paraspinal gutter depend on knowing what lies medial (spinalis and semispinalis) versus lateral (iliocostalis and levator scapulae), and this posterior perspective keeps those relationships clear while respecting the segmental layout of the cervical vertebrae.
