Posterior cervical anatomy is centered on the spinous processes of C2 through C7, with the occipital bone superiorly and the superior angles and spines of the scapulae appearing laterally as landmarks. The spinalis cervicis is positioned in the medial column of the erector spinae, deep to trapezius and typically deep to or blending with semispinalis cervicis, running from upper thoracic spinous processes toward the spinous processes of the axis and midcervical vertebrae. Medial to it lie the nuchal ligament and the midline osseous ridge of the cervical vertebrae, while laterally the transversospinalis group and the levator scapulae region define the posterior neck contour. Bone context usually includes the posterior arch region of C1, the bifid spinous processes of midcervical vertebrae, and the cervicothoracic junction inferiorly. A posterior view clarifies how spinalis cervicis contributes to cervical extension and ipsilateral stabilization, functions that matter when evaluating whiplash-associated disorders and chronic axial neck pain patterns tied to extensor overuse.
