Description
Emerging from the midline nuchal region, the splenius capitis forms a paired, obliquely oriented muscle mass on the posterior neck, running superolaterally from the lower cervical and upper thoracic spinous processes toward the mastoid process and the lateral superior nuchal line of the occipital bone. Medially it borders the cervical spinous processes and ligamentum nuchae, while laterally it approaches the posterior triangle region deep to the superficial cervical fascia. Deep to the more superficial trapezius, the splenius capitis overlies semispinalis capitis and the upper cervical vertebrae, with the superior fibers tucking under the occipital bone. Clear bony landmarks anchor the view. Posterior exposure matters because splenius capitis is a common pain generator in cervicogenic headache and occipital neuralgia patterns, where myofascial trigger points can refer pain toward the mastoid, temporal region, and vertex. For posterior cervical approaches, it also functions as a practical layer marker: splitting the midline raphe and reflecting trapezius reveals the splenius group before deeper dissection toward the suboccipital triangle and the posterior arch of C1. Orientation is everything.
