Description
Arising from the spinous processes of the upper thoracic vertebrae and the lower cervical vertebrae, the splenius capitis forms paired, oblique muscle bellies that ascend superolaterally across the posterior neck to insert on the mastoid process of the temporal bone and the lateral third of the superior nuchal line of the occipital bone. In lateral profile, its fibers lie posterior to the cervical vertebral column and deep to the trapezius, creating a broad band between the midline nuchal ligament and the more lateral shoulder girdle. Inferiorly, the muscle mass blends into the posterior cervicothoracic region above the upper ribs, with the scapulae and clavicles providing bony reference points. Splenius capitis is a primary landmark when teaching the posterior cervical muscle layers and when orienting to deeper structures such as semispinalis capitis and the suboccipital triangle. Clinically, this anatomy sits directly in the field of posterior cervical approaches and targeted injections for cervicogenic headache and occipital neuralgia, where referral patterns and tenderness often localize along the nuchal line and mastoid attachment.
