Description
Posterolateral neck anatomy is presented on a male skeletal framework, with the cervical vertebrae aligned beneath the occiput and the upper thorax partially included for orientation. Along the posterior cervical spine, the semispinalis cervicis would be expected deep to the splenius cervicis and trapezius, spanning superiorly from the upper thoracic transverse processes toward the spinous processes of C2 to C5. Lateral to this column, the levator scapulae is shown as a paired strap muscle running inferolaterally from the posterior tubercles of C1 to C4 to the superior angle and medial border of the scapula. Tight geometry. Clavicles, scapulae, and upper ribs supply clear bony landmarks for the shoulder girdle. Semispinalis cervicis matters because its segmental attachments make it a primary contributor to cervical extension and contralateral rotation, and its deep position places it in the surgical corridor during posterior approaches to the cervical spine. In C2 pedicle or pars fixation, for example, detaching and reflecting deep paraspinal layers while preserving midline attachments can influence postoperative axial neck pain and extensor weakness; semispinalis cervicis is part of that conversation. The juxtaposition with levator scapulae also helps explain why scapular elevation and ipsilateral neck side-bending can coexist in muscular imbalance, yet represent different functional compartments (deep transversospinales versus lateral cervical stabilizers).
