Description
Posterior cervical musculature is presented in lateral profile, with the spinalis cervicis emphasized along the posterior column of the neck. The muscle belly lies deep to trapezius and adjacent to the semispinalis cervicis, running superiorly and slightly medially from the spinous processes of the upper thoracic vertebrae (typically T1 to T5) to insert on the spinous processes of the mid cervical vertebrae (most often C2 to C4). Anterior to these fibers, the cervical vertebral bodies and their transverse processes with transverse foramina define the bony contour of the cervical lordosis, while the skull, mandible, and upper thoracic cage establish craniovertebral and cervicothoracic context. Functionally, spinalis cervicis contributes to extension of the cervical spine and, with unilateral contraction, subtle ipsilateral rotation and lateral flexion, actions that become clinically relevant when you assess posterior neck pain after whiplash or prolonged forward head posture. This lateral perspective helps differentiate true posterior extensor columns from the more anterolateral scalenes, a common point of confusion in teaching and in procedural planning. It also supports discussion of the cervicothoracic junction as a transition zone where extensor overuse and facet-mediated pain often localize, and where posterior approaches must respect layered muscle planes to reduce postoperative spasm.
