Description
Running obliquely from the midline of the posterior cervical spine toward the mastoid region, the splenius capitis is presented in a posterior view of the male neck with the adjacent splenius cervicis continuing inferiorly toward the upper thoracic spinous processes. Medial attachments align with the ligamentum nuchae and the spinous processes of C7 to T3 or T4, while the muscle fibers course superolaterally to insert on the mastoid process of the temporal bone and the lateral third of the superior nuchal line. Overlying trapezius has been removed or reduced to expose the splenius layer, and the cervical vertebrae provide clear midline orientation. Short and clear. Clinically, this view maps directly to common pain generators in cervicogenic headache and posterior neck strain, where splenius capitis trigger points can refer pain to the occiput and behind the eye and can be confused with greater occipital neuralgia. The oblique fiber direction also explains the combined actions seen on exam: bilateral extension of the head and neck, and unilateral ipsilateral rotation with lateral flexion, patterns often reproduced after whiplash injury. Posterior exposure makes the relationship to the nuchal ligament and upper cervical spinous processes easy to teach before discussing deeper suboccipital muscles and the vertebral artery in the suboccipital triangle.
