Description
Seen from the lateral neck and upper shoulder, the longissimus cervicis is positioned posterior to the cervical transverse processes and deep to the more superficial posterior cervical musculature, running as an elongated column within the erector spinae group. Its fibers course superiorly from upper thoracic transverse processes toward the posterior tubercles of the mid cervical vertebrae, lying medial to the levator scapulae and posterior to the scalene compartment. Anteriorly, the anterior scalene is highlighted, descending from cervical transverse processes to the first rib, with the sternocleidomastoid expected to overlie the scalene region more superficially. Clean layering matters. For teaching, this lateral arrangement clarifies the functional split between posterior extensors and lateral flexors of the cervical spine (longissimus cervicis) and the anterolateral rib elevator and neck flexor (anterior scalene).
