Description
Running deep along the posterolateral cervical spine, the longissimus cervicis appears as a vertical column of erector spinae muscle extending between upper thoracic transverse processes and the posterior tubercles of the mid cervical transverse processes. Laterally, it sits deep to trapezius and often just posterior to the plane of the levator scapulae, while the splenius cervicis and capitis occupy a more superficial, oblique layer over the posterior neck. Anterior to these posterior compartment muscles, the sternocleidomastoid and scalene group frame the lateral neck, and a superficial blue venous channel in the carotid region suggests the course of the internal jugular vein beneath the investing fascia. Depth matters. This lateral, subcutaneous perspective clarifies how the longissimus cervicis contributes to ipsilateral cervical extension and side bending while remaining clinically easy to confuse with the more superficial splenius and levator scapulae on palpation. Neck pain patterns after whiplash and sustained head-forward posture often involve overactivity and trigger points in the deep cervical extensors, and this view helps separate the longissimus cervicis from the accessory nerve dependent trapezius that frequently becomes secondarily guarded.
