Description
Running longitudinally along the posterolateral thorax, the longissimus thoracis occupies the intermediate column of the erector spinae mass, positioned lateral to spinalis and medial to iliocostalis. From a lateral perspective in a male torso, its muscle belly tracks superior to inferior over the thoracic cage, with fibers oriented cranially toward the upper thoracic region and caudally toward the thoracolumbar junction. Deep to it lie the thoracic transverse processes and ribs, and inferiorly the muscle approaches the posterior iliac region via the common erector spinae tendon. Clinically, this angle clarifies why longissimus thoracis becomes a frequent pain generator and target in thoracolumbar myofascial syndromes, where trigger points can refer discomfort laterally around the ribs or into the posterior flank. The relationship to the rib angles and transverse processes matters during thoracic paraspinal injections and radiofrequency procedures, since a slightly more lateral needle trajectory risks contacting rib or pleura, while a medial drift brings you closer to the zygapophysial joints. It is a big extensor. It also contributes to ipsilateral lateral flexion and helps stabilize the thoracic spine during respiration and loaded lifting.
