Description
Seen from a posterior perspective with the skin removed, the vertebral column sits on the midline as the organizing axis for the paraspinal musculature. Longissimus thoracis forms the prominent intermediate column of the erector spinae, lateral to spinalis and medial to iliocostalis, with its fibers running longitudinally from the lumbar region toward the thoracic transverse processes and ribs. Deeper to the erector spinae, the transversospinales group, including semispinalis and multifidus, spans from transverse processes superomedially to spinous processes, creating the oblique fiber pattern that hugs the laminae. More superficially, trapezius and latissimus dorsi bracket the thoracolumbar fascia and scapular region, while splenius capitis and splenius cervicis occupy the posterolateral neck. Layering matters here. This is the arrangement encountered when reflecting trapezius and latissimus dorsi to reach the intrinsic back muscles during a posterior spinal approach or when teaching how the thoracolumbar fascia compartmentalizes the erector spinae. Multifidus atrophies early in chronic low back pain and after lumbar surgery, and its segmental attachments explain why even small disruptions around the facet joints can alter stability. Palpation and dry needling targets also map cleanly onto these columns, but only if you appreciate what lies medial versus lateral to the spinous processes. Use this plate in gross anatomy and kinesiology courses to teach intrinsic versus extrinsic back muscles, in orthopedic or neurosurgical education to orient trainees for posterior exposure, or in rehabilitation texts discussing erector spinae mechanics and multifidus dysfunction.
