Description
Running longitudinally on either side of the thoracic vertebral column, the longissimus thoracis is highlighted within the erector spinae mass, positioned lateral to spinalis thoracis and medial to iliocostalis. From this posterior perspective, its muscle belly and tendonous slips track superiorly toward the thoracic transverse processes and adjacent ribs, crossing segmentally behind the rib cage. Surrounding landmarks include the spinous processes along the midline, paired ribs curving laterally, and the scapulae resting posterolaterally over the upper thorax. Bony context is clear. Clinically, this view supports teaching of paraspinal compartment anatomy and the functional line of pull for thoracic extension and ipsilateral lateral flexion, where longissimus thoracis acts with iliocostalis during loaded trunk tasks. Palpation and needling targets often lie 2 to 4 cm lateral to the spinous processes, a region frequently implicated in mechanical thoracic back pain, myofascial trigger points, and postural overuse syndromes in athletes and desk-bound patients. For spine surgery orientation, the illustration helps explain the posterior midline approach where the erector spinae are elevated from the spinous processes and laminae to expose the facet joints and transverse processes while minimizing iatrogenic muscle injury.
