Description
Running longitudinally on either side of the vertebral column, the longissimus thoracis is highlighted as the intermediate column of the erector spinae, positioned lateral to spinalis and medial to iliocostalis. From a posterior full-body perspective, its muscle belly parallels the thoracic spinous processes while sending slips laterally toward the rib angles and superiorly toward the thoracolumbar junction. Surrounding osseous landmarks remain visible in situ, including the occiput and cervical vertebrae superiorly, the scapulae overlying the posterior ribs, and the sacrum and posterior iliac crests inferiorly. Limb bones are included for orientation, but the emphasis stays on the paraspinal corridor. For teaching spinal mechanics, this view clarifies how longissimus thoracis contributes to trunk extension and ipsilateral lateral flexion across multiple motion segments, with force transmitted through the thoracolumbar fascia into the posterior pelvis. Palpation and needling targets often track this muscle along the thoracic transverse processes and rib angles, where myofascial pain patterns can mimic costovertebral or facet-mediated pain. It also maps cleanly to procedural anatomy: the longissimus lies deep to the erector spinae plane block injection site and forms part of the muscular envelope encountered during midline posterior approaches for thoracic instrumentation.
