Description
Beneath the reflected skin of the posterior trunk, the longissimus thoracis is presented as the dominant intermediate column of the erector spinae, running longitudinally from the lumbosacral region toward the upper thoracic spine. Medial to it, the spinalis forms a narrower band adjacent to the spinous processes, while the iliocostalis lies more laterally, tracking toward the rib angles. Superficial landmarks such as the trapezius superiorly and latissimus dorsi inferolaterally frame the field, and the vertebral column sits deep on the midline. Clear layering. Longissimus thoracis matters because it is the most substantial extensor of the thoracic spine and a consistent landmark when teaching back muscle compartmentalization and dorsal ramus innervation patterns. In posterior approaches to the thoracolumbar spine, splitting the erector spinae between the longissimus and multifidus (the classic Wiltse interval) can reduce midline soft-tissue disruption and helps explain why postoperative pain differs between midline and paramedian dissections. Clinically, localized myofascial pain over the paraspinal gutter often tracks with overuse or spasm of the erector spinae, and this view anchors palpation relative to the spinous processes and rib angles.
