Description
Seen from a lateral perspective, the levator scapulae forms a vertical, column-like strap along the posterolateral cervical region, descending from the transverse processes of C1 to C4 to attach on the superior angle and medial border of the scapula. Superiorly it sits deep to the sternocleidomastoid and posterior to the carotid sheath, while posteriorly it relates to the splenius cervicis and the upper fibers of trapezius as it approaches the scapular spine. Inferiorly, its scapular insertion lies medial to the shoulder girdle and just superior to the origin of rhomboid minor. Orientation is clear: cervical origin proximal, scapular insertion distal. Clinically, this view is the one you want when teaching scapular elevation and downward rotation mechanics, because the fiber direction explains why trigger points in levator scapulae often refer pain to the posterolateral neck and superior medial scapular border. It also frames a common scenario in whiplash and postural strain, where sustained scapular depression or forward head posture loads the muscle near its C1 to C2 attachments.
