Description
Presented from a lateral aspect of the male crus, the superficial posterior compartment is dominated proximally by the lateral head of gastrocnemius, with soleus visible deep and inferior as it funnels into the calcaneal (Achilles) tendon posterior to the ankle. Along the lateral border of the fibula, fibularis (peroneus) longus lies more superficial and proximal, while fibularis brevis sits deeper and becomes more prominent distally as both tendons course posterior to the lateral malleolus beneath the superior and inferior fibular retinacula. The fibula tracks posterior-lateral to the tibia, and the lateral contour of the talus and calcaneus anchors the distal relationships. Lateral leg anatomy is where ankle mechanics and tendon pathology intersect. This perspective clarifies how the fibularis tendons turn sharply around the lateral malleolus, a common site for peroneal tenosynovitis, tendon subluxation when the superior fibular retinaculum is torn, and split tears of fibularis brevis associated with recurrent inversion sprains. It also reinforces the compartmental arrangement relevant to exertional compartment syndrome and to safe surgical approaches that protect nearby neurovascular structures, including the course of the sural nerve in the posterolateral ankle.
