Description
Running from the medial femoral condyle to the medial malleolus, the tibia anchors the deep posterior compartment, with tibialis posterior and flexor digitorum longus lying immediately posterior to the tibial shaft while flexor hallucis longus sits deeper and more lateral. Superficial to these, the medial head of gastrocnemius and the soleus form the bulk of the calf, their aponeuroses converging distally toward the calcaneal (Achilles) tendon. Across the medial knee, the sartorius, gracilis, and semitendinosus tendons approach the anteromedial proximal tibia as the pes anserinus, and at the ankle the posterior tibial neurovascular bundle courses behind the medial malleolus with the tendons that continue into the foot. Medial anatomy of the crus matters because it concentrates structures that are palpated, stressed, and surgically approached every day, from the medial tibial border to the tarsal tunnel. The relationship of the posterior tibial tendon to the medial malleolus and navicular is central to posterior tibial tendon dysfunction and adult acquired flatfoot, while crowding of the tibial nerve and companion vessels beneath the flexor retinaculum explains classic tarsal tunnel syndrome patterns. Proximally, the pes anserinus and adjacent medial collateral ligament region is a frequent source of pain in runners and patients with knee osteoarthritis. Landmarks stay consistent.
