Description
Seen from the lateral neck, the thyroid lobe wraps anterolaterally around the upper trachea, while the parathyroid glands appear as small ovoid bodies tucked on the posterior surface of the thyroid, close to the posteromedial border. Superior and inferior parathyroids are positioned at different craniocaudal levels, with the superior gland usually nearer the level of the cricoid cartilage and the inferior gland trending closer to the inferior pole of the thyroid. Superiorly, the laryngeal framework (thyroid and cricoid cartilages) caps the airway, and the tracheal rings extend inferiorly in the midline. A lateral perspective matters because it matches the surgeon’s mental map during thyroidectomy and parathyroid exploration, where glands can hide behind the thyroid capsule and near the expected course of the recurrent laryngeal nerve in the tracheoesophageal groove. Hypocalcemia from inadvertent devascularization or removal of parathyroid tissue remains a practical complication to teach, and this orientation helps explain why careful capsular dissection and preservation of the inferior thyroid artery branches are emphasized.
