Description
Posterior cervical anatomy frames the thyroid gland from behind, with four parathyroid glands rendered as small ovoid nodules on the posterior thyroid capsule, classically a superior pair near the cricothyroid junction and an inferior pair closer to the lower pole. Medial to the thyroid, the cervical trachea and upper esophagus sit in the visceral compartment, while the cervical vertebral column lies posteriorly in the midline. Laterally, the carotid sheath contents (common carotid artery continuing as the internal carotid, internal jugular vein, and vagus nerve) run vertical, and the sternocleidomastoid and scalene muscle groups define the superficial and lateral boundaries of the neck. A posterior perspective matters because it tracks the relationships surgeons depend on during thyroidectomy and parathyroid exploration: the recurrent laryngeal nerve ascends in or near the tracheoesophageal groove and approaches the larynx close to the inferior thyroid artery, both adjacent to typical parathyroid locations. Preserve the parathyroid vascular pedicles, and postoperative hypocalcemia becomes less likely. This view also supports teaching the embryologic migration pattern that explains ectopic inferior glands, often encountered along the thyrothymic tract into the superior mediastinum.
