Description
Seen from a lateral perspective, the pelvis and proximal thigh frame the posterior femoral cutaneous nerve as it exits the pelvic cavity and descends into the posterior compartment of the thigh. Superiorly, lumbar vertebrae and the sacrum provide the proximal reference, while the nerve’s course is oriented inferiorly and slightly posterior to the hip joint as it passes deep to the gluteal region. The femoral head sits within the acetabulum anteriorly, and the nerve tracks posterior to the proximal femur toward the hamstring group, remaining superficial to the deep posterior musculature compared with the adjacent sciatic nerve. Clear bony landmarks include the iliac crest and ischium. Orientation of the posterior femoral cutaneous nerve matters when distinguishing posterior thigh pain from true sciatic distribution, since the nerve supplies cutaneous sensation over the inferior buttock and posterior thigh via its perineal and inferior cluneal branches. That distinction shows up in clinic. In the lateral approach to the hip, posterior acetabular exposure, or deep intramuscular injections in the gluteal region, this nerve’s relationship to the ischial tuberosity and posterior femur helps explain sensory complaints after trauma, iatrogenic injury, or compression near the infrapiriform region where it travels close to the sciatic nerve.
