Description
Running inferiorly from the sacral plexus in the pelvis, the posterior femoral cutaneous nerve emerges near the greater sciatic foramen and descends deep to gluteus maximus, then tracks along the posterior thigh in close company with the sciatic nerve. From a lateral view, its course can be followed superficial to the hamstring compartment, with cutaneous branches spreading toward the posterolateral thigh and the inferior clunial nerves turning superiorly across the lower buttock. Bony landmarks such as the greater trochanter, femur, and the lateral contours of the tibia and fibula anchor the nerve’s relationship to the iliotibial tract, biceps femoris, and the lateral head of gastrocnemius. Clear topography. Clinically, this is the nerve you think of in posterior thigh and perineal sensory complaints when motor strength is preserved, since it is purely cutaneous and often overlooked beside the sciatic nerve. Its proximity to the ischial tuberosity and the inferior border of gluteus maximus makes it susceptible during posterior hip approaches, intramuscular injections placed too inferiorly and medially, and procedures around the proximal hamstrings, including endoscopic repairs. The lateral perspective also helps distinguish posterior femoral cutaneous neuralgia from lateral femoral cutaneous nerve entrapment (meralgia paresthetica), which localizes to the anterolateral thigh. Use this plate in gross anatomy and neuroanatomy teaching to trace sacral plexus branches into named cutaneous territories, and in operative or sports medicine texts discussing posterior hip portals, proximal hamstring pathology, and nerve block planning around the gluteal region. It also fits well in patient education materials explaining why posterior thigh numbness can occur without weakness after hip or hamstring interventions.
