Description
Prominent posterior landmarks include the paired gluteus maximus muscles overlying the hip, with the hamstring compartment (biceps femoris laterally, semitendinosus and semimembranosus medially) descending from the ischial region into the posterior thigh. Inferior to the gluteal fold, the perforating cutaneous nerve emerges around the lower margin of gluteus maximus and turns superficially into the proximal posterior thigh, where it lies posterior to deeper neurovascular structures. Deep and lateral to this small sensory branch, the sciatic nerve courses inferiorly from the gluteal region, continuing between the hamstrings as it approaches the popliteal fossa. A small nerve. A consequential one. Orientation at the inferior border of gluteus maximus matters because it is where sensory nerves to the buttock and posterior thigh can be injured by posterior hip approaches, deep intramuscular gluteal injections, or trauma around the ischial tuberosity. This posterior view clarifies how a cutaneous branch can appear deceptively close to the sciatic nerve on surface inspection while remaining a separate structure that pierces fascia near the gluteal crease, a common source of diagnostic confusion during evaluation of posterior thigh dysesthesia. Pain patterns here can mimic proximal hamstring tendinopathy or referred pain from lumbosacral radiculopathy.
