Arising from the parotid and masseteric fascia over the lateral cheek, the risorius runs anteriorly and slightly inferiorly to blend with the modiolus at the angle of the mouth. In lateral profile, its thin strap-like belly lies superficial to the buccinator and crosses anterior to the parotid gland region, with the zygomatic arch and body of the mandible providing bony context deep to the soft tissues. Posteriorly the muscle relates to the anterior border of the masseter, while anteriorly it approaches the orbicularis oris at the oral commissure. The skull, mandible with dentition, and upper cervical spine sit in the background as fixed landmarks for orientation. Functionally, risorius retracts the oral commissure laterally, contributing to the grimace and the social smile, and it is one of the more variable muscles of facial expression in both size and presence. That variability matters in the operating room and in the clinic, because the region is traversed by buccal branches of the facial nerve and the facial artery and vein, and iatrogenic weakness can blunt commissure retraction after parotidectomy or facelift dissection along the parotid fascia.
