Description
Seen from the lateral aspect, the male sacrum appears as a wedge-shaped fusion of five sacral vertebrae (S1 to S5) sitting inferior to L5 and superior to the coccyx. The promontory of S1 projects anteriorly at the lumbosacral junction, while the concave pelvic surface lies anterior and the roughened dorsal surface lies posterior. Inferiorly, the sacral apex narrows to meet the base of the coccyx at the sacrococcygeal joint. Clear margins of the alae and the superior articular processes are expected at the upper end, with the coccyx curving anteroinferiorly. A true lateral perspective matters because it clarifies sagittal alignment at the lumbosacral junction, where shear forces concentrate and spondylolisthesis most often occurs. Landmarks such as the S1 promontory, sacral slope, and sacrococcygeal curvature are the same references used when assessing pelvic incidence and planning fixation, including iliosacral screw trajectories and lumbosacral fusion constructs. This is also where many fractures declare themselves on radiographs. Subtle step-offs can be missed. Educators can drop this into pelvic anatomy and spine modules to reinforce the transition from lumbar vertebrae to fused sacrum and the relationship of sacrum to coccyx in male pelvic morphology.
