Presented from the anterior aspect, the male sacrum is oriented with the promontory superiorly at the pelvic brim and the apex tapering inferiorly toward the coccyx. The anterior sacral foramina align in paired columns lateral to the midline, separated by the transverse ridges that mark fused sacral segments. A horizontal fracture line crosses the sacral ala near the arcuate line region, interrupting the continuity between the sacral body and the lateral mass adjacent to the sacroiliac articular surface. Instability is implied where the fracture approaches the pelvic brim. Anterior sacral fractures at the ala matter because they often accompany pelvic ring trauma, and they change the mechanics of load transfer from L5 through the sacral promontory into the ilium. Small shifts here can destabilize the posterior pelvic ring even when the anterior pubic rami injury draws most attention. For fixation planning, the relationship of the fracture to the anterior sacral foramina and the sacral ala corridor guides safe trajectories for iliosacral or transsacral screws and helps anticipate L5 or sacral nerve root irritation in foraminal zone injuries. A common pitfall. Underestimating displacement.
