Description
Seen in strict lateral profile, the superior palpebral venous arcade forms a curved blue plexus within the upper eyelid, arching parallel to the superior tarsal plate and following the eyelid margin toward the lateral canthus. Small venules converge into this loop from the pretarsal and preseptal tissues, then course posteriorly toward the orbital rim, where drainage routes communicate with the superficial facial veins and deeper orbital veins. Red arterial companions track nearby across the forehead and periorbita, while the zygomatic bone, maxilla, and mandible provide fixed bony landmarks for the periocular vasculature. Teeth and the alveolar processes remain visible as orientation points in the facial skeleton. Lateral emphasis on the superior palpebral venous arcade matters because eyelid veins sit at the junction between the superficial facial venous network and valveless orbital pathways. Infection or thrombophlebitis from the eyelid can spread via periorbital venous connections toward the superior ophthalmic vein and cavernous sinus, a classic concern in preseptal versus orbital cellulitis. Surgical planning also depends on this anatomy. Upper blepharoplasty, ptosis repair, and lateral canthotomy or cantholysis can produce troublesome bleeding if the venous arcade and its tributaries are not anticipated along the superior lid and canthal region.
