Description
Seen from a lateral perspective, the lumbar vertebral bodies stack vertically with bluish intervertebral discs interposed, while the pedicles and laminae form the posterior wall of the vertebral canal and the intervertebral foramina open laterally between adjacent vertebrae. Posterior to the vertebral bodies, the superior and inferior articular processes meet to form the zygapophysial (facet) joints, their articular surfaces oriented to guide flexion and extension while limiting rotation. Along the posterolateral margins, bands consistent with the anterior and posterior longitudinal ligaments and segmental capsular thickening blend into the periarticular tissues. Paraspinal muscle slips occupy the most posterior and lateral plane. A clarification matters here: transverse costal facets (costal facets on transverse processes) belong to thoracic vertebrae and articulate with the tubercle of a rib at the costotransverse joint, whereas the lumbar transverse processes lack rib facets. In the lumbar region, lateral pain generators are more often the facet joints and their capsules, the exiting nerve roots within the foramen, or disc-related osteophytes at the posterior vertebral margins. This lateral view supports teaching and clinical correlation for lumbar facet arthropathy, foraminal stenosis, and planning trajectories for facet joint injections or medial branch blocks. A common trap is mislabeling thoracic costotransverse anatomy as lumbar. Use this artwork for gross anatomy and musculoskeletal modules when contrasting thoracic costotransverse joints with lumbar zygapophysial joints, or for spine texts discussing segmental motion, degenerative facet disease, and lateral recess or foraminal compromise on imaging.
