Description
Oriented from the anterior aspect, the vertebral bodies stack superior to inferior with intervertebral discs interposed, while the pedicles and laminae frame the vertebral canal posteriorly. Laterally, the transverse processes project from the vertebral arch, and their transverse costal facets (foveae costales processus transversi) appear as paired articular surfaces positioned posterolateral to the vertebral bodies and lateral to the pedicles. Costal facets sit on the transverse processes. Facet (zygapophysial) joints are also indicated at the junction of superior and inferior articular processes, though they lie more posterior than the costal articulations. Focusing on the transverse costal facets matters because they localize the costotransverse joints that couple thoracic spine motion to rib mechanics during respiration. In thoracic trauma and inflammatory arthropathies, these joints can be pain generators, and their proximity to the transverse process makes them a practical target for image guided injection or for correlating with thoracic paravertebral block landmarks. From an anatomic teaching standpoint, the presence of transverse costal facets distinguishes typical thoracic vertebrae from cervical and lumbar levels, and the facets’ position explains why rotation is relatively free in the thoracic region compared with flexion and extension.
