Description
Posterolateral thoracic vertebrae are arranged in vertical sequence, with the vertebral bodies bearing superior and inferior costal demifacets (foveae costales) as shallow concavities for rib head articulation. Matching transverse costal facets (foveae costales processus transversi) sit lateral to the pedicles on the transverse processes, positioned posterior to the vertebral body and anterior to the spinous process. Intervertebral discs separate adjacent bodies, while longitudinal ligamentous bands track superior to inferior along the column. Small articular facet surfaces at the zygapophysial joints appear posterior to the vertebral body, framing the intervertebral foramina where thoracic spinal nerve roots would exit. Clear landmarks. Costovertebral and costotransverse joints govern thoracic cage mechanics, so precise depiction of these facets matters when teaching why thoracic motion couples rotation and lateral flexion differently than in the lumbar spine. Degenerative change at the costovertebral articulations can mimic pleuritic pain, and osteophytes at the demifacets or zygapophyseal joints may narrow the intervertebral foramen, contributing to thoracic radiculopathy. This view also supports procedural planning: surgeons and interventionalists rely on the relationship between transverse processes, rib heads, and facet joints when approaching thoracic paravertebral blocks, medial branch blocks, or posterolateral decompression.
