Description
Running along the plantar aspect of the lateral foot, the adductor digiti minimi is presented in profile as an elongated intrinsic muscle belly adjacent to the fifth metatarsal and the proximal phalanx of the little toe. Superior to it, the distal tibia and fibula frame the talocrural (ankle) joint as they articulate with the talus, while the calcaneus sits posterior and inferior as the main weightbearing tarsal. Anteriorly, the tarsometatarsal region leads into the metatarsals and phalanges, with joint capsules and ligamentous contours indicated around the ankle, subtalar, and midfoot articulations. Orientation is lateral, so the fibula is the dominant malleolar landmark. Plantar intrinsic anatomy is easiest to lose in a sea of extrinsic tendons, so a clean lateral view helps you place adductor digiti minimi relative to the lateral longitudinal arch and the fifth ray. This is the region approached during lateral midfoot and hindfoot exposures, and it is also the corridor where peroneal tendons course behind the lateral malleolus and where swelling after an inversion injury can obscure palpation of the cuboid, calcaneocuboid joint line, and base of the fifth metatarsal. Small muscle, real function. Expect it to be discussed with other hypothenar analogs of the foot when teaching toe alignment and lateral forefoot stability.
