Description
Presented from a lateral section perspective, the cerebrum is opened to expose the deep gray nuclei of the basal ganglia, dominated by the lentiform nucleus with the putamen positioned lateral to the globus pallidus. Medial to the lentiform complex, the thalamus sits in the diencephalon, and the striatum is represented by the caudate nucleus tracking along the lateral ventricle in a more superior and anterior course relative to the thalamus. The internal capsule logically intervenes between these masses, separating the lentiform nucleus laterally from the caudate nucleus and thalamus medially. Posterior and inferior in the same cut, the brainstem transitions toward the spinal cord, with the cerebellum lying dorsally. A lateral section like this is the workhorse for teaching basal ganglia circuits because it clarifies the spatial logic behind the direct and indirect pathways: cortical input to striatum, pallidal output, and thalamocortical relay. It also maps cleanly onto common clinical patterns, including lacunar infarcts of the lenticulostriate arteries affecting the posterior limb of the internal capsule with contralateral pure motor hemiparesis, and movement disorders where degeneration of the substantia nigra pars compacta reduces dopaminergic drive to the striatum. Small lesion, big deficit. This view offers the clearest perspective on why. Use it in neuroanatomy labs and exam prep to anchor relationships among the striatum, globus pallidus, and thalamus, and in neurology or neurosurgery teaching materials that discuss stroke localization, hemiballismus, Parkinson disease, and targets for deep brain stimulation.
