Description
Oriented in lateral profile, the distal tibia and fibula frame the talocrural joint superiorly, with the talus seated between the malleoli and the calcaneus forming the posterior pillar of the hindfoot. Anterior to the calcaneus, the cuboid and navicular are positioned along the lateral and medial columns respectively, while the cuneiforms, metatarsals, and phalanges extend distally to outline the forefoot. The extensor hallucis brevis muscle belly lies on the dorsolateral aspect of the foot, arising from the superolateral calcaneus and coursing anteriorly to a tendon that heads medially toward the hallux, deep to the extensor digitorum brevis and inferior to the extensor retinacula. Clear relationships. Lateral visualization matters because extensor hallucis brevis is a frequent landmark when teaching dorsal foot compartments and when orienting dorsal approaches to the midfoot and tarsometatarsal region. Painful swelling over the dorsolateral calcaneus and sinus tarsi region can be confused with tendinopathy of the extensor tendons or with subtalar pathology, and this perspective helps separate a short, intrinsic dorsum muscle belly from longer extrinsic tendons that traverse the ankle. It also supports discussion of the deep fibular (peroneal) nerve and dorsalis pedis artery course on the dorsum, structures that can be irritated by dorsal incision placement or tight footwear.
