Description
Seen in lateral section within the bony orbit, the globe sits anterior to the orbital apex while the annulus tendineus communis at the posterior orbit gives rise to the four rectus muscles, which course forward to insert on the sclera. Superior and inferior rectus lie above and below the globe, lateral rectus occupies the lateral wall, and medial rectus runs along the medial side toward the nasal aspect, partially obscured by the sectioned skull. The superior oblique tracks anteromedially toward the trochlea before redirecting posterolaterally across the superior surface of the globe, while the inferior oblique originates from the anterior medial orbital floor and passes laterally and posteriorly beneath the globe. Muscle paths are shown in direct relationship to the orbital bones and the confined retrobulbar space. A lateral orbital cut is the clearest way to teach the functional “cone” formed by the rectus muscles and why crowding at the apex matters. Optic nerve compression from thyroid eye disease, orbital cellulitis, or retrobulbar hemorrhage after trauma is often discussed in terms of limited expansion within this cone and the proximity of the rectus origins to the optic canal and superior orbital fissure. Strabismus surgery and forced-duction testing also depend on understanding where each muscle can be isolated and how the obliques change torsion as well as elevation and depression.
