Description
Viewed from the left lateral aspect of the male pelvis, the rectum and distal sigmoid colon occupy the posterior compartment, with the rectal plexus draped along the posterolateral rectal wall within the mesorectal fascia. Fine autonomic fibers converge from superiorly placed pelvic splanchnic nerves (S2 to S4) and sympathetic contributions via the hypogastric nerves, then redistribute as a dense perirectal network that tracks inferiorly toward the anorectal junction. Small nodular and lobulated pericolic structures along the bowel surface are consistent with appendices epiploicae and regional lymph nodes, positioned more laterally and anteriorly relative to the rectal muscularis. A lateral perspective matters because it clarifies how the pelvic autonomic nerves course close to the rectum before entering the plexus, a relationship that drives functional outcomes after total mesorectal excision for rectal cancer. Preserve this plane and you reduce postoperative urinary retention and erectile dysfunction; violate it and the cost is predictable. The same anatomy underlies neuropathic pelvic pain and defecatory disorders, where irritation of the inferior hypogastric plexus and its rectal branches can mimic primary colorectal disease.
