Description
Laterally, the superior extensor retinaculum appears as a dense transverse band of deep fascia spanning the anterolateral ankle, coursing from the distal fibula toward the lateral surface of the calcaneus. Deep to this retinaculum, the extensor tendons pass anterior to the tibiotalar joint and align toward the dorsum of the foot, with the tendon of fibularis (peroneus) tertius lying most laterally and extensor digitorum longus just medial to it. The retinaculum sits superficial to the extensor tendon sheaths and anterior joint capsule, acting as the proximal roof of the osseofibrous tunnels that keep these tendons closely apposed to the malleolar region. Orientation is clear. Lateral malleolus provides the bony reference. Clinically, this lateral perspective matters because extensor tendon bowstringing and tenosynovitis often declare themselves where the superior extensor retinaculum fails to restrain the tendons during dorsiflexion, especially after ankle sprain with anterolateral soft tissue injury.
