Description
Viewed from the posterior aspect with a left-sided emphasis, the inferior (lower, basal) lobes of the right and left lungs occupy most of the thoracic cavity below the oblique fissures, extending inferiorly toward the costodiaphragmatic recess. On the left, the inferior lobe lies posterior to the lingula and inferior to the superior lobe, with its medial surface approaching the posterior mediastinum and vertebral column. Laterally, the basal segments contour the inner aspect of the lower rib cage, while the posterior basal region projects closest to the posterior thoracic wall. Color coding differentiates the lobi inferiores cleanly. Posterior lung anatomy is where physical examination and many bedside correlations live: lower lobe airspace disease and pleural fluid both declare themselves at the back, not the front. This view clarifies why dependent atelectasis and aspiration pneumonia often localize to the posterior basal segments in supine patients, and why dullness to percussion and reduced breath sounds are typically sought over the lower posterior rib spaces when a pleural effusion is suspected. It also helps orient the clinician to the oblique fissure, a key landmark when interpreting chest radiographs or describing lobar distribution on CT reports. Suitable for gross anatomy labs, respiratory system modules, and radiology teaching files that need a straightforward posterior map of the lung bases and their relationship to the lower thoracic cage.
