Description
Posterior aspects of the right and left lungs are rendered with emphasis on the inferior (lower, basal) lobes and their interlobar surfaces. The oblique fissure defines the superior boundary of each inferior lobe, while the smooth visceral pleura forms the glistening interface where lobes appose one another. Inferiorly, the caudal lung bases broaden toward the diaphragmatic surface, and laterally the basal parenchyma sweeps toward the costal surface, clarifying the three-dimensional contour of the lower lobes. A posterior orientation is the practical way to think about dependent disease. Lower lobe atelectasis and aspiration pneumonia often track into the posterior basal segments when a patient is supine, and pleural effusions layer posteriorly and inferiorly before they blunt the costophrenic angle on radiographs. Fissural anatomy matters too, because fluid can collect in the oblique fissure (a fissural pseudotumor) and lower lobectomy follows the fissure plane, with dissection proceeding along visceral pleura where the lobes separate rather than through aerated parenchyma. Use this asset in thoracic anatomy teaching to reinforce lobe boundaries, pleural reflections, and why “basal” and “caudal” are more than directional terms when interpreting chest X-ray, CT, and ultrasound of the posterior lung bases. It also fits surgical atlases and perioperative education on right and left lower lobectomy, segmentectomy planning, and common postoperative patterns of lower lobe collapse.
