Description
Centered in the upper abdomen and lower thorax, the distal esophagus passes through the esophageal hiatus of the diaphragm to meet the gastric cardia at the gastroesophageal junction. A localized hiatus hernia displaces the proximal stomach, typically the cardia and adjacent fundus, superiorly through the diaphragmatic opening into the posterior mediastinum, lying posterior to the heart and between the lungs. The right and left crura frame the hiatus, with the herniated gastric pouch positioned more superior than the remaining body and antrum of the stomach, which continue inferiorly toward the pylorus. Surrounding viscera provide orientation, including the left lobe of liver anterior and superior to the stomach and the spleen lateral to the gastric fundus. Clinical relevance hinges on how this anatomy disrupts the anti-reflux barrier created by the diaphragmatic crura and the lower esophageal sphincter acting in concert at the hiatus. Sliding hiatal hernia commonly accompanies gastroesophageal reflux disease, while a paraesophageal component raises concern for gastric volvulus, obstruction, or strangulation when the fundus herniates alongside the esophagus and becomes trapped above the diaphragm. Correlating the displaced cardia with adjacent thoracic structures also supports interpretation of barium swallow studies and endoscopy reports, where the position of the Z-line relative to the diaphragmatic pinch is a key landmark. Small changes matter.
