Description
Centered on the gastroesophageal junction, the distal esophagus descends to the gastric cardia and proximal fundus, with a portion of stomach displaced superiorly through the esophageal hiatus of the diaphragm, consistent with an established hiatal (diaphragmatic) hernia in an adult male. The stomach is opened to reveal prominent gastric rugae along the body and antrum, while the lesser curvature tracks toward the pylorus and duodenum inferiorly. Superior and leftward, the left lobe of liver frames the proximal stomach; laterally, the spleen sits against the fundus, and the pancreas lies posterior-inferior to the stomach along the duodenal sweep. Hiatal herniation matters because even a modest cranial migration of the cardia can disrupt the angle of His and the lower esophageal sphincter zone, a common substrate for reflux esophagitis, Barrett esophagus, and aspiration-related symptoms. It also sets the stage for the surgical anatomy of repair: the right and left crura forming the hiatus, the anterior and posterior vagal trunks on the esophagus, and the relationship of the fundus used in a Nissen or partial fundoplication, where a too-tight wrap can produce postoperative dysphagia. A true paraesophageal component carries a different risk profile, including gastric volvulus and strangulation, so clear depiction of the herniated cardia versus fundus is often what clinicians and educators want.
