Description
Medial orientation of the right lung places the hilum front and center, with the right main bronchus posterior to the pulmonary artery and the superior and inferior pulmonary veins occupying the more anterior and inferior portions of the root. Along the mediastinal surface, the superior vena cava lies anterolateral to the trachea and typically leaves a vertical sulcus, while the arch of the azygos vein curves superiorly toward the hilum to join the SVC. Inferiorly and posteriorly, the esophagus and thoracic vertebral column create subtle impressions as the pulmonary ligament descends below the hilum toward the diaphragm. Clear fissural anatomy often remains visible on the right, separating superior, middle, and inferior lobes even from a medial perspective. This is the view you want when teaching or planning around the bronchopulmonary root, because small changes in relative position matter when you pass instruments, place staplers, or interpret cross-sectional imaging. Lymph nodes at the right paratracheal and subcarinal stations sit adjacent to these structures, and their enlargement can distort the normal hilar relationships seen here. It also frames common clinical patterns, such as right hilar fullness from bronchogenic carcinoma, azygos lobe variants, or pulmonary embolus effects on the pulmonary arterial tree.
