Medial forearm skin has been removed to expose the extensor digitorum muscle belly along the posterior compartment, running distally from the lateral epicondyle region toward the dorsum of the hand. From this ulnar sided perspective, the ulna sits medial to the radius, and the extensor digitorum lies posterolateral to the ulnar shaft, with its fascial plane separating it from deeper extensors. Distally, the extensor digitorum tendons converge and pass beneath the extensor retinaculum to reach the dorsal expansions of digits two through five, where they sit superficial to the metacarpals and phalanges. Understanding this relationship matters when you teach or plan approaches around dorsal wrist compartments and the common extensor origin. Lateral epicondylitis often involves the extensor origin complex, and distinguishing the extensor digitorum from neighboring extensor carpi radialis brevis or extensor carpi ulnaris helps explain pain patterns and grip weakness.
