Description
Arising from the posterior surface of the tibia inferior to the soleal line, the flexor digitorum longus occupies the deep posterior compartment and tracks distally along the posteromedial border of the leg, just posterior to the tibia and medial to the fibula. From a medial perspective, its muscle belly is seen deep to the gastrocnemius-soleus complex, narrowing into a tendon that courses behind the medial malleolus under the flexor retinaculum, adjacent to the posterior tibial artery and tibial nerve. Distal to the ankle, the tendon continues into the plantar foot, crossing the flexor hallucis longus at the knot of Henry and dividing into four slips to the distal phalanges of toes 2 through 5. Bony landmarks include the knee region at the proximal tibia and the ankle mortise with the talus and calcaneus. Medial visualization matters because the flexor digitorum longus is one of the structures confined within the tarsal tunnel, where crowding, tenosynovitis, or space-occupying lesions can irritate the tibial nerve and mimic plantar fasciitis with medial ankle pain and plantar paresthesias. It also clarifies why surgeons harvest or transfer the flexor digitorum longus tendon in posterior tibial tendon dysfunction, routing it to support the medial longitudinal arch while preserving toe flexion through the remaining digital flexors.
