Rendered in posterior anatomical position, the flexor digitorum longus (FDL) occupies the deep posterior compartment of the leg, arising from the posterior surface of the tibia inferior to the soleal line and running distally along the medial side of the calf. Its muscle belly lies deep to gastrocnemius and soleus and posterior to the tibia, while the tendon courses inferiorly behind the medial malleolus to enter the plantar foot, where it will divide to the lateral four digits. Medially, the tendon tracks close to the tibialis posterior tendon, and more laterally it relates to flexor hallucis longus as the two cross in the plantar midfoot. Deep. That medial retromalleolar course makes FDL clinically specific: it passes through the tarsal tunnel with the posterior tibial neurovascular bundle, a corridor implicated in tarsal tunnel syndrome and in surgical decompression planning. Orthopaedic foot and ankle surgeons also reference FDL when treating posterior tibial tendon dysfunction, where an FDL tendon transfer may augment a failing tibialis posterior to restore the medial longitudinal arch. The posterior view clarifies why distal symptoms can reflect proximal mechanics, since the tendon’s line of pull couples toe flexion with plantarflexion and inversion.
