Description

Running deep in the posterior compartment of the male leg, the flexor digitorum longus arises from the posterior tibia inferior to the soleal line and descends medial to the fibula, tucked beneath the soleus and gastrocnemius. Its muscle belly narrows into a tendon that passes posterior to the medial malleolus, accompanied by the tibialis posterior and flexor hallucis longus in the tarsal tunnel, then continues into the plantar foot to reach the lateral four toes. Medially the tibial shaft dominates the field, while the fibula sits lateral and slightly posterior, giving a clear bony reference for compartment boundaries. Clinically, this posterior view is the one you want when teaching the order of tendons behind the medial malleolus and the structures vulnerable in tarsal tunnel syndrome, where swelling or varicosities can compress the tibial nerve adjacent to the flexor digitorum longus tendon sheath. It also supports surgical planning around posteromedial ankle approaches and tendon transfers, since the flexor digitorum longus is commonly harvested to augment a dysfunctional posterior tibial tendon in adult acquired flatfoot, with careful attention to preserving the neurovascular bundle.
