Description
Arising from the plantar surface of the cuboid and adjacent tendon sheath of the fibularis (peroneus) longus, the flexor digiti minimi brevis lies in the third plantar layer along the lateral border of the foot, just inferior to the bases of the fourth and fifth metatarsals. From a medial oblique perspective, its short belly is seen deep to the superficial plantar tissues, coursing distally toward the fifth metatarsophalangeal joint where its tendon inserts on the lateral side of the base of the proximal phalanx of the little toe. The tibia and fibula remain proximal reference points, with the talus seated between their malleoli, and the tarsals, metatarsals, and phalanges aligned distal to the ankle mortise. Small joint surfaces and cartilage at the ankle, tarsometatarsal, and metatarsophalangeal articulations frame the muscle’s position relative to the bony lever system it acts upon. Flexor digiti minimi brevis is an intrinsic muscle that often gets overlooked in favor of the long flexors, yet its line of pull at the fifth ray matters when you teach lateral column mechanics and toe purchase during gait. Pain along the lateral forefoot can involve pathology at the fifth metatarsophalangeal joint, including bunionette deformity, plantar plate irritation, or iatrogenic imbalance after corrective procedures, and appreciating where this muscle runs helps explain why symptoms localize plantarly and laterally rather than dorsally. Short. Compact. Deep. That combination is why it is easily missed in superficial dissections and even in quick clinical sketches.
