Shown from the medial aspect, the distal femur and proximal tibia form the tibiofemoral joint, with the patella positioned anterior to the femoral trochlea and its medial margin aligned toward the inner joint line. The inferomedial genicular nerve descends along the medial side of the knee, running in close proximity to the periosteum and the medial capsular tissues as it approaches the inferomedial quadrant of the joint. Articular cartilage caps the femoral condyle and tibial plateau, framing the nerve’s terminal articular distribution in relation to the weightbearing surfaces. Course and relationship matter here because the inferomedial genicular nerve is a key target in genicular nerve radiofrequency ablation for osteoarthritis-related anterior and medial knee pain, and its position relative to the medial tibial plateau and adjacent genicular vessels guides safe needle placement. Small errors in mediolateral depth can shift the lesion away from the articular branch or place it too close to vascular structures. Landmarks are unforgiving.
