Description
Arising within the plantar venous plexus of the sole, the lateral plantar veins course anteriorly along the lateral longitudinal arch, paired with the lateral plantar artery, and then turn proximomedially toward the medial malleolus. From this medial perspective, the veins can be followed deep to the plantar aponeurosis and adjacent to the calcaneus, cuboid, and bases of the fourth and fifth metatarsals before converging toward the tarsal tunnel region. Posterior to the medial malleolus, their drainage pathway aligns with the neurovascular bundle, where the posterior tibial veins receive tributaries that have traversed the plantar foot. Relationship drives interpretation. Venous anatomy in the plantar foot matters when you are thinking about compartmental pressure, edema, or any process that narrows the fibro-osseous tarsal tunnel. Entrapment syndromes most often focus on the tibial nerve, but space-occupying lesions such as varicosities, venous malformations, or post-traumatic hematoma in the venous plexus can contribute to tarsal tunnel syndrome and complicate decompression. This orientation also supports teaching the deep venous return from the lateral sole into the posterior tibial system, a pathway relevant when correlating to Doppler ultrasound mapping or planning incisions that respect the neurovascular bundle behind the medial malleolus.
