Description
Running along the posteromedial calf, the plantaris appears as a small fusiform muscle belly near the lateral supracondylar line of the femur and the oblique popliteal ligament, then continues distally as a long, slender tendon. From this medial perspective, the tendon can be followed inferiorly as it crosses between gastrocnemius and soleus, tracking toward the calcaneal region alongside the Achilles (calcaneal) tendon. Medial landmarks typically visible in this relationship include the medial head of gastrocnemius, the broad soleus deep to it, and the tibia positioned medial to the fibula. Small but unmistakable. Clinical relevance comes from variability. The plantaris may be absent, duplicated, or have a tendon that blends with the calcaneal tendon or inserts independently on the calcaneus, distinctions that matter when interpreting calf pain after a sudden sprinting start, the common setting for so-called tennis leg involving plantaris rupture or a medial gastrocnemius tear. Surgeons also encounter the plantaris tendon as a thin structure that can be harvested for grafting, and its proximity to the medial Achilles region keeps it in the conversation around midportion Achilles tendinopathy and paratendinous irritation.
