Description
Plantar dissection of the male forefoot centers on the flexor hallucis brevis within the central compartment of the sole, lying inferior to the first metatarsal head and deep to the plantar aponeurosis and superficial digital flexor tendons. Its muscular belly divides distally into medial and lateral tendinous slips that course to either side of the flexor hallucis longus tendon, then insert on the medial and lateral sides of the base of the proximal phalanx of the hallux. Paired sesamoid bones are typically embedded within these slips at the level of the first metatarsophalangeal joint, acting as pulleys as the tendons pass plantar to the metatarsal head. Close neighbors include the tendon of flexor hallucis longus centrally, the adductor hallucis oblique head laterally, and the abductor hallucis along the medial border. Understanding this plantar relationship matters when teaching or operating around the first metatarsophalangeal joint, where sesamoiditis, sesamoid fracture, and plantar plate pathology can mimic hallux rigidus or metatarsalgia. The split insertion of flexor hallucis brevis helps explain why sesamoid excision, bunion surgery (hallux valgus correction), or approaches for turf toe injuries can destabilize hallux flexion and alter the line of pull on the proximal phalanx.
