Plantar dissection exposes the flexor digitorum brevis in the central sole, lying deep to the plantar aponeurosis and superficial to the tendons of flexor digitorum longus. Its fleshy belly arises from the medial process of the calcaneal tuberosity and fans distally into four slips that course toward digits 2 through 5, positioned between the abductor hallucis medially and abductor digiti minimi laterally. Near the proximal phalanges, each slip divides into two tendinous bands, creating a window for the flexor digitorum longus tendon to pass through before the brevis inserts on the sides of the middle phalanx. A clean landmark. That split matters because it explains the layered mechanics of toe flexion at the metatarsophalangeal and interphalangeal joints, and it clarifies why distal interphalangeal flexion is dominated by flexor digitorum longus while flexor digitorum brevis contributes mainly at the proximal interphalangeal level. Clinically, plantar incisions for foreign body removal, fibromatosis, or approach to the plantar plates risk violating the brevis slips and the adjacent common plantar digital nerves and vessels that run between muscular planes. The relationship to the calcaneal tuberosity also makes it a useful reference when discussing heel pain syndromes, where overload of the plantar fascia and intrinsic muscles can coexist and mimic one another on exam. Course directors in gross anatomy and podiatric medicine can use this perspective to teach the first layer of the plantar foot and the tendinous chiasm that permits flexor digitorum longus transit to the lateral four toes, while surgical authors can pair it with plantar approach figures for tendon repair, diabetic foot debridement, or intrinsic muscle sparing flaps.
